DANCE NORTHSIDE CONCERT PERMISSION FORM 2025

Our 2025 concert is on Saturday the 5th of July at The Canberra Theatre Centre.

Please use this form to let us know whether or not your dancer will be performing. Submit one response per student.

Terms and conditions

Students choosing to participate in the concert (ie. those who answered 'Yes' to the above) understand that they:
- are committed to performing on Saturday 5th July, and are required to be at Canberra Theatre Centre in the morning and afternoon/evening on this date;
- will be photographed and filmed during the performance, and these images will be used for promotional purposes on Dance Northside's website and social media;
- incur a $45 performance fee, plus an additional $15 costume fee for each additional performing class they are enrolled in.

Students choosing not to perform in the concert (ie. those who answered 'No' to the above) are welcome to attend classes during Term 2 and learn the routine alongside performing students. However, please be aware that due to costuming constraints and Canberra Theatre Centre's policy, we cannot accommodate changes of mind after the end of Week 2 (Saturday 10th May). This means dancers cannot decide at a later date that they would actually like to perform, because they will not have been allocated a costume or included in the performer numbers we provide to the theatre.
